    Position Summary: We are looking for a Per-Diem Hospice Medical Social Worker to provide crucial support to our patients and their families. The ideal candidate will have prior experience working in hospice care, possess exceptional interpersonal skills and community resources, be committed to upholding the highest standards of patient care and support, and speaks Korean.

    Responsibilities:

    Conduct psychosocial assessments of patients and their families to identify emotional, social, and financial needs through in person visits and phone calls.

    Develop and implement individualized care plans in collaboration with the interdisciplinary team to address identified needs and goals.

    Provide counseling and emotional support to patients and families facing end-of-life issues, grief, and bereavement.

    Assist patients and families in accessing community resources and support services.

    Document all assessments, interventions, and interactions on HospiceMD in accordance with organizational and CHAP standards.

    Collaborate with interdisciplinary team members, including physicians, nurses, chaplains, and volunteers, to ensure comprehensive care delivery.

    Participate in staff IDG meetings to discuss patient and family care plans.

    Requirements:

    Bachelor's or Master's degree in Social Work from an accredited institution. Medical Social Worker (MSW) or Licensed Social Worker (LSW) preferred.

    Experience working in hospice care.

    Korean speaking highly preferred.

    Excellent communication and interpersonal skills.

    Valid driver's license and reliable transportation for local travel.

    Willing to travel to Los Angeles, Orange, and Riverside counties.

    Ability to work independently and as part of a multidisciplinary team.

    Compassionate and empathetic approach to patient care.

    Strong organizational and time-management skills.
