What You’ll Do

As a Hospice Medical Social Worker, you will:

• Assess: Evaluate the psychosocial needs of patients and families, sharing insights with the interdisciplinary team to ensure holistic care.
• Counsel: Offer guidance and support to patients and families facing stress and coping challenges, including crisis intervention when needed.
• Educate: Assist families with advance directives and provide clarity on patient rights and available resources.
• Collaborate: Build and maintain relationships with community agencies to enhance patient care and support.
• Participate: Contribute to personalized care plans and attend interdisciplinary meetings to address the psychosocial dimensions of terminal illness.

Qualifications

• Education: * Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education, or * Bachelor’s degree in Social Work with additional qualifications per Medicare guidelines.
• Experience: At least one year in healthcare; experience in home health or hospice is preferred.
• Skills: Strong verbal and written communication, excellent organizational skills, and a deep understanding of hospice philosophy and end-of-life care.
• License: Current state licensure, a valid driver’s license, and access to a reliable, insured vehicle.
