Are you a dedicated physician committed to enhancing the quality of life for patients and families during end-of-life care? Aachele Home Care and Hospice is seeking a compassionate and experienced Hospice Medical Director to lead our clinical team in providing exceptional hospice services.

Responsibilities:  Provide medical leadership and guidance to the hospice interdisciplinary team.  Actively participate in interdisciplinary team meetings bi-weekly.  Conduct initial and follow-up patient assessments as needed.  Certify and re-certify patients’ eligibility for hospice care, including completing appropriate documentation.  Provide direct medical care and consultation for hospice patients as required.  Oversee the development and implementation of patient care plans in collaboration with the care team.  Conduct patient evaluations and certify terminal illness for hospice eligibility.  Serve as a liaison between hospice staff, attending physicians, and specialists to ensure coordinated care.  Ensure compliance with Medicare, Medicaid, and accreditation standards.

Qualifications:  MD or DO with an active and unrestricted medical license in Georgia.  Board certification in hospice and palliative medicine (preferred) or related field.  Minimum of 2 years of experience in hospice, palliative care, or a related specialty. (preferred)  Knowledge of hospice regulations and standards of care.
