Responsibilities: • Perform thorough patient assessments, including monitoring vital signs and reviewing medical histories, to gain a comprehensive understanding of their condition and needs. • Develop personalized care plans considering medical evidence, cultural aspects, and ethical considerations to deliver individualized care to each patient. • Administer medications and treatments safely as prescribed, following all protocols and ensuring patient safety at all times. • Educate patients and their families on health conditions, treatment plans, and preventive measures to empower active participation in their care. • Collaborate with physicians and healthcare professionals to ensure a holistic approach to patient care, promoting optimal outcomes. • Maintain accurate health records and care plans in compliance with regulations to ensure continuity of care. • Perform clinical duties such as IV therapy, wound care, and catheterization with professionalism and adherence to best practices. • Provide emotional and psychological support to patients and families, especially during challenging times, to promote comfort and well-being. • Adhere strictly to infection control protocols to prevent disease transmission and maintain a safe environment for patients and staff. • Stay updated on medical advances and maintain necessary certifications to continuously improve knowledge and skills. • Contribute to quality improvement initiatives through active participation in audits and safety evaluations to enhance care delivery. Qualifications: • To be considered for the role of Hospice Nurse Case Manager in Freehold, NJ, US, a valid RN license issued by the state of New Jersey is mandatory. • The candidate must possess current certifications in BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support). • While previous experience in hospice care is preferred, it is not a strict requirement for this position. • Essential qualifications include excellent communication skills, strong interpersonal abilities, and a proven track record of effective teamwork. • The candidate must demonstrate a commitment to delivering compassionate care to patients and their families. • Attention to detail and the ability to remain composed in stressful situations are critical attributes for this role.
