Responsibilities: • Conduct thorough assessments of patients' medical conditions, symptoms, and pain management needs in the hospice setting in NJ. • Develop personalized care plans in coordination with the hospice interdisciplinary team and the patient's family. • Regularly update and review care plans to adapt to changing patient needs and maintain the highest quality of care. • Monitor patients' vital signs, treatment responses, and overall well-being to ensure optimal care outcomes. • Collaborate with healthcare professionals like physicians and palliative care specialists to enhance patient comfort and pain management. • Offer emotional and spiritual support and counseling to both patients and their families during the end-of-life process. • Respect and honor patients' spiritual beliefs and cultural practices to foster a sense of peace and acceptance. • Facilitate regular interdisciplinary team meetings to discuss patient care plans and address any concerns. • Coordinate services provided by the hospice team members, including social workers, chaplains, and home health aides. • Engage with external healthcare providers and community resources to ensure comprehensive support for patients and their families. • Educate patients and families about hospice philosophy, the end-of-life journey, and available resources. • Provide guidance on pain management techniques and coping strategies for emotional challenges. • Maintain accurate patient records, documenting all care provided and ensuring effective communication with patients and families. • Ensure compliance with relevant regulations, policies, and procedures in hospice care in NJ. Qualifications: • MUST BE AN ACTIVE RN LICENSE IN NJ • A minimum of 2-3 years of clinical nursing experience is required, with a preference for backgrounds in hospice, palliative care, or related fields. • Essential to have a profound understanding of hospice philosophy and the principles of end-of-life care. • Strong communication and interpersonal skills are a necessity to engage effectively with patients, families, and the interdisciplinary team. • Demonstrates empathy, compassion, and sensitivity when supporting individuals and families through grief and loss. • Capable of working both independently and collaboratively within a team, showcasing robust organizational and time-management skills. • Proficiency in utilizing electronic health records and other pertinent healthcare software applications is expected. Compensation: $45 - $48 hourly
