    Job Summary

    We are seeking a Registered Nurse to join our team at Angel Care Hospice and Palliative. In this role, you will primarily be creating and managing a plan of care for individual patients. Our hospice team is looking for a Registered Nurse (RN) who operates with great attention to detail and a big heart. Our patients reside in their own residence or residential facility throughout Los Angeles County. Our RNs take pride in creating personalized care plans for patients and are able to observe and communicate effectively in the patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as excellent communication and time management skills. Angel Care Hospice & Palliative is recognized in the community for providing outstanding end-of-life care for our patients and being a dependable resource for the families and caregivers. Our Hospice RNs work as part of an interdisciplinary team including LVNs, CHHAs, Physicians, Social Workers, Chaplains and other support staff.     Responsibilities

    Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans.

    Participate in IDT meetings to discuss patient progress and assess level of needs.

    Communicate effectively with hospice team regarding patient needs.

    Administer medications and monitor patients for side effects or reactions

    Document any changes in patient conditions and report on changes timely and accurately.

    Promote high quality and effective interventions and outcomes.

    Maintain accurate and timely records of patient care and discuss observations with the supervisor.

    Adhere to professional standards as outlined by protocols, rules and regulations.

    Qualifications

    Currently licensed Registered Nurse (RN) in state of California

    Previous experience as a Registered Nurse is preferred

    Excellent interpersonal and client care skills

    Valid driver’s license

    First aid/CPR certification required

    Compassionate and a team player

    Excellent organizational and time management skills
