Position Overview: We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated team, to manage patients in Los Lunas, Belen, and the South Valley ABQ. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. Responsibilities:
Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
Develop and implement individualized care plans in collaboration with the interdisciplinary team.
Provide skilled nursing care, including symptom and medication management.
Coordinate resources, referrals, and care services for patients and families.
Offer education and emotional support to patients and their loved ones regarding end-of-life care and planning.
Qualifications:
Current licensure as a Registered Nurse (RN) in New Mexico.
Experience in hospice or end-of-life care settings preferred.
Strong critical thinking and assessment skills.
Compassionate and empathetic approach to patient care.
Excellent communication and teamwork skills.
