Hospice Social Worker

We have recently engaged with a very progressive, well-run run, financially stable hospice organization in the Savannah, GA area. They are a not-for-profit organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.

Our client is offering the following benefits:

• Annual Salary: $48,000 -$55,000+ Based On Experience

Requirements:

• Master’s Degree in Social Work or LCSW
• Demonstrated ability to provide quality patient care.
• Performs Initial and Ongoing Assessment of the Patient and Family’s Psychosocial Status and Assists in Accessing Resources for Social, Financial, and Community Needs for Adequately Managing Care and Dealing with Death
• Develop a Plan of Care That Optimizes the Patient and Family's Ability to Cope with the Psychosocial Impact of Illness and Impending Death
• Offer Support, Guidance, and Counseling in Order to Diminish Stress and Enhance Emotional Well Being

Responsibilities:

• M – F (8:00AM to 5:00PM)
• No Call
• The Social Worker Will Maintain a Case Load of About 35-40 Patients
• 3-4 Patients Per Day
