    We’re looking for a compassionate Hospice Social Worker to support patients and families through one of life’s most challenging times. In this role, you’ll provide emotional and practical support, helping to address social, emotional, and environmental needs. As part of a care team, you’ll work with nurses, doctors, and others to create personalized care plans. Your work will include counseling, connecting families with resources, and guiding them through the hospice process. Responsibilities: • Provide Emotional Support and Counseling to Patients and Families. • Help individuals navigate the emotional challenges of end-of-life care, ensuring they feel heard, understood, and supported during this sensitive time. • Conduct Psychosocial Assessments. • Evaluate patients’ and families’ emotional, social, and environmental needs to develop personalized care plans that address their unique circumstances. • Coordinate Community Resources and Services. • Connect families with financial aid, transportation, respite care, or other essential resources to ease their burden and improve their quality of life. • Collaborate with the Interdisciplinary Team. • Work closely with nurses, doctors, chaplains, and others to create holistic care plans that meet physical, emotional, and spiritual needs. • Educate Patients and Families about Hospice Services. • Guide them through the hospice process, explaining what to expect and addressing questions to reduce uncertainty and fear. • Advocate for Patients’ Needs and Preferences. • Ensure patients’ wishes are respected and that they receive care aligned with their cultural, spiritual, and personal values. • Provide Bereavement Support. • Offer grief counseling and resources to families after a loved one’s passing, helping them navigate their loss and healing process. Qualifications: • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in New Jersey. • Required to ensure compliance with state regulations and to provide professional-level care. • Experience in hospice, palliative care, or healthcare settings preferred. • Familiarity with end-of-life care ensures an understanding of the unique needs of patients and families. • Strong interpersonal and communication skills. • Ability to build trust, convey empathy, and communicate effectively with patients, families, and team members. • Knowledge of community resources and support systems. • Helps connect patients and families with appropriate services to address their specific needs. • Ability to work collaboratively in an interdisciplinary team. • Ensures holistic care by integrating perspectives from healthcare, social work, and other disciplines. Compensation: $75,000 - $80,000 yearly
