Summary Description
The House Lead at New Transitions will oversee daily operations within a residential setting, ensuring high-quality care, coordination, and supervision of Direct Support Professionals (DSPs). Under the supervision of the Program Director, the House Lead will also provide hands-on care to residents, promote a positive living environment, and ensure adherence to all standards and protocols. This position requires a current CNA certification and completion of a 3-4 day Designated Coordinator/Designated Manager (DC/DM) training through STAR Services, once the program dates are set.
Job Requirements:
Licensure and Certification:
Valid Certified Nursing Assistant (CNA) certification.
Completion of a 3-4 day Designated Coordinator/Designated Manager (DC/DM) training through STAR Services (dates to be determined).
Education:
High school diploma or equivalent.
Additional training or coursework in healthcare, human services, or management is preferred.
Experience:
Minimum of 1-2 years of experience in a healthcare or residential care setting.
Previous experience in a supervisory or leadership role is an advantage.
Experience working with individuals with disabilities or special needs is highly preferred.
Skills:
Strong leadership and organizational abilities.
Excellent communication and interpersonal skills.
Proficiency in electronic health records (EHR) systems and documentation.
Ability to work independently and as part of a team.
Physical stamina to perform duties requiring hands-on care and mobility assistance.
Background Check:
Must pass a criminal background check.
Must have a clean driving record and a valid driver’s license.
Job Responsibilities:
Resident Care and Support:
Provide direct care to residents, assisting with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and medication administration.
Monitor and document residents’ health status, noting any changes and promptly reporting to the Program Director.
Support residents in achieving their personal goals, ensuring alignment with individual care plans.
Leadership and Supervision of DSPs:
Supervise and guide DSPs in delivering high-quality care, setting a positive example, and fostering a supportive work environment.
Coordinate schedules to ensure adequate staff coverage, including delegating tasks as needed.
Provide feedback, mentorship, and regular check-ins with DSPs to address challenges and promote professional development.
Training and Compliance:
Complete the required DC/DM training through STAR Services and apply the learned skills within the role.
Train DSPs on care protocols, safety procedures, and facility policies, ensuring they adhere to all guidelines.
Conduct regular in-services and ongoing training sessions for DSPs to maintain compliance and improve care standards.
Household Operations and Maintenance:
Oversee daily household operations, ensuring the residence remains clean, safe, and welcoming.
Coordinate with vendors and the Program Director for any repairs or maintenance needs.
Manage and procure household and resident supplies, keeping inventory stocked.
Documentation and Reporting:
Maintain accurate and timely records in compliance with company policies and regulatory requirements.
Document residents’ progress on care plans, incident reports, and daily logs.
Report any significant changes in residents’ conditions or household issues to the Program Director.
Quality Assurance and Safety:
Conduct regular assessments to ensure the facility is meeting health and safety standards.
Enforce emergency preparedness protocols and conduct regular drills.
Address any immediate safety concerns, prioritizing the well-being of both residents and staff.
Communication and Collaboration:
Act as a liaison between the Program Director, staff, residents, and families, fostering clear and positive communication.
Attend and contribute to meetings with the Program Director and other team members.
Facilitate communication among DSPs to ensure smooth handovers and continuity of care.
Crisis Management and Conflict Resolution:
Lead in managing any crises within the residence, using established protocols to ensure the safety of all residents and staff.
Address conflicts among staff or residents professionally, involving the Program Director as necessary.
