    Job Summary:

    Multicultural Care Center LLC is in search of Housing Case Manager for its Housing Stabilization Service (HSS) program. This position provides screening and intakes, housing search and placement, rehousing, and landlord recruitment to help single adults find and keep housing in Hennepin and Ramsey Counties.

    The ideal candidate will have the skills to build relationships with private landlords and property managers; provide case management (assessment, plan development, and monitoring; referrals; coordination and advocacy with necessary resources and benefits), housing tenancy support, and direct service. The preferred candidate will use a person-centered, harm reduction, and housing-first approach to partner with participants from diverse communities who have experienced homelessness and behavioral health challenges.

    Job duties and responsibilities include:

    Housing intake, search, and placement.

    Develop individualized housing search and placement plans for initial or rehousing placements.

    Locate new landlords; build positive relationships with private landlords and property managers.

    Connect households to security deposits, moving costs, furnishings, basic needs and emergency resources.

    Assist with arranging for and supporting the details of the move. Perform Habitability Inspections and required recertification processes.

    Provide tenancy-sustaining services and case management with a small caseload (understanding the lease, communicating with the landlord, managing neighbor conflicts, paying rent on time, and addressing other behaviors that may jeopardize housing).

    Utilize skills and community resources to provide crisis prevention and intervention.

    Support participant health and wellness through connections to physical, mental health, and recovery resources.

    Assist participants to maintain or increase income through benefits assistance or involvement with volunteering, education, and employment activities.

    Help participants navigate transportation needs, including access to bus tokens, gas cards and driving residents to appointments in personal vehicle.

    Documenting and case noting daily activities and tasks performed

    using Calendar to show appointments and home visits scheduled

    Education and Experience:

    Bachelor Degree in Human Services, Social Work, or related field, and experience with target population may be considered as a substitute for some education. Bachelor's Degree preferred.

    2-3 years’ experience in providing services to individuals or families with behavioral health challenges and or in a supportive housing environment. Prefer experience in housing search and placement.

    Salary: $21.00-$22.00/hr. DOQ
