Job description
One Health is currently looking for an experienced, talented and dynamic Intake Coordinator. Job Responsibilities
Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.
Works with physicians, nurses, social workers and discharge planners to establish a coordinated plan of care.
Obtains and documents prior authorization for services from insurance providers.
Ability to demonstrate compassion and courtesy when dealing with difficult situations.
Capable of working under pressure and time constraints including overtime to complete daily assessments.
Ability to use sound judgment and tactful approach when dealing with co-workers, staff and the public.
Must possess the ability to use and apply clinical knowledge to the referral process.
Required Skills
Minimum of one year of experience in intake coordinating.
Self-starter who enjoys working in a fast-paced environment with the ability to multi-task.
Excellent problem-solving, observation, written and verbal communication skills.
Knowledge of overall home health management including admission criteria, nursing process and insurance limitations.
Basic knowledge of insurances including Medicare/Medi-Cal and HMO limitations.
Effective interpersonal communication skills.
Fluent knowledge of English, both verbal and written.

Pay: $22.00 - $26.00 per hour
