Sereno Clinic is seeking a compassionate and skilled IOP/PHP Facilitator to join our clinical team. The IOP/PHP Facilitator will be responsible for leading therapeutic groups, providing direct support to patients, and ensuring the delivery of quality care in our structured outpatient programs. The ideal candidate will be dedicated to helping individuals on their journey to recovery, fostering an environment of safety, respect, and emotional well-being.
Key Responsibilities:
Lead and facilitate therapeutic groups for individuals in our IOP/PHP programs, focusing on mental health, substance use, coping skills, and relapse prevention.
Develop and implement group curriculum in alignment with evidence-based practices and the needs of the patient population.
Provide individual support to patients during group sessions, offering guidance, feedback, and encouragement.
Collaborate with the clinical team to assess patient progress and provide input for treatment planning.
Ensure all documentation is accurate, timely, and compliant with regulatory and licensing requirements.
Conduct crisis intervention when necessary and escalate issues to appropriate clinical staff.
Maintain a therapeutic and supportive environment that encourages patient participation and engagement in treatment.
Attend team meetings and contribute to the development of individualized care plans.
Provide family support and education as needed.
Qualifications:
Experience: Minimum of 2 years of experience in a therapeutic setting, with a strong understanding of IOP/PHP treatment models.
Licensure: Must hold a valid, applicable state license (e.g., ADT, LGPC, LCSW, CSC-AD, etc.).
Skills:
Strong facilitation and group leadership skills.
Knowledge of evidence-based therapeutic approaches, including CBT, DBT, and motivational interviewing.
Ability to work effectively in a team environment and collaborate with multidisciplinary staff.
Excellent communication and interpersonal skills.
Empathy, compassion, and the ability to connect with patients in an engaging, non-judgmental manner.
Preferred: Experience working with diverse populations and a strong understanding of trauma-informed care.
