Are you a skilled and compassionate Physical Therapist looking to make a positive impact on the lives of patients in need of rehabilitation services? If so, we invite you to join our team as an In-Home Outpatient (IHOP) Physical Therapist at RVNAhealth. We are a dynamic and patient-centered healthcare facility committed to delivering high-quality care to our community. This is a full-time position will cover the Central territory including Ridgefield, Danbury, Bethel, South Norwalk and Greenwich.
Position Overview: As an IHOP Physical Therapist, you will play a vital role in providing outpatient physical therapy services to a diverse patient population including post surgical total joint replacements. You will direct, supervise, evaluate, and provide, physical therapy services to patients accepted for care by RVNAhealth as prescribed by the attending physician, focusing on both in-home and Assistive Living Facility. The therapist is responsible to the Director of Rehabilitative Services for fulfilling the needs of the agency in the area of his/her expertise.
Responsibilities:
Comprehensive evaluation of patient's level of function and participation in development of the total patient care plan with, patient, family, and physician.
Identification of patient and family needs for other home health services and referral for same when needed.
Able to treatment of patients per protocols and requirements of PT practice act per State of Connecticut
Participation in case conferences as needed.
When working within ALF’s, communicate with Wellness Center, patient and family as needed.
Instruction of patient, family and other agency health care personnel in the patient's treatment regime when indicated.
Assists patients to reach a level of function to enable them to remain safely in their home environment.
Obtains written physician's order for Physical Therapy every sixty (60) days, more often if necessary.
Performs the safety evaluation prior to the patients total joint replacement.
Performs subsequent visit following surgery.
Experience:
Graduate with a baccalaureate degree of an accredited school of Physical Therapy approved by the American Medical Association Council of Medical Education with collaboration with the American Physical Therapy Association.
Current license to practice as a Physical Therapist in Connecticut.
At least two (2) years' clinical experience.
Knowledge of physical therapy methods with skill in casework in the part B home setting.
Knowledge and understanding of the principles of Part B home care and billing.
