Above It All is seeking contractual Intake & Assessment Specialists to provide bio psychosocial assessments, crisis interventions, develop Individualized Rehabilitation Plans and offer referrals and resources for clients and families enrolling in services.

Responsibilities

Conduct confidential, quality intake and Biopsychosocial Assessments.

Develop and review Individualized Rehabilitation Plans.

Based on the Assessments, make referrals to the most appropriate programs and agencies.

Working within a multidisciplinary context, participate in professional meetings, maintain accurate and protected medical info, and provide reports as warranted for funding sources. Crisis intervention as needed.

Job Requirements

Master's Level licensed mental health professional (LGSW, LGPC, LMSW,CPRP, LCSW-C, LCPC)

Two years of experience working with a program that provides services to individuals with emotional and behavioral disabilities.

Have experience with developing treatment plans and completing assessments.

Must hold a valid driver's license with acceptable driving record and provide own, insured transportation.
