Role Summary
The Intake Care Coordinator is often the first point of contact for new and returning clients. This
position is responsible for responding to and screening incoming clients to identify service needs. This
provider will engage as a member of a collaborative team. With a Trauma Informed approach, this
position will be a representative for the organization and how we offer integrated services for recovery
and mental health.
Essential Duties
Time
40% Direct Client Care: Provide trauma informed services to clients primarily on the phone and in the
office. Provide basic information on programs and services. Assist clients in completing initial
required documentation. Deliver positive results as evidenced by utilization of outcome
measures. Perform direct service at 100% of billable expectations.
40%Coordination: With the Clinical Manager, manage schedule of Diagnostic Assessments. Prepare
initial required documentation for incoming clients. Coordinate services with clients, staff, and
community partners.
10% Documentation: Provide accurate, concise, and concurrent documentation according to program
procedures and policies. Including, but not limited to obtaining client demographics and basic
clinical information.
5% Professional Development: Seek and secure professional development consistent with licensure
and credential requirements for personal and organizational growth.
5% Miscellaneous Programmatic Duties and Intra-Organizational Support:
• Capitalize on opportunities to support programmatic needs across the organization
• Participate in supervision and team meetings, work groups, task forces, or other general
efforts that add value to the success of the organization and programming
• Provide case management and/or crisis intervention/support as needed
• Other duties as needed/assigned
Requirements:
Pay Rate 20.00 per hour
