    THE ROLE

    We are seeking an Intake Coordination Specialist – Social Care Networks to play a vital role in launching Anthos|Home's participation in two Social Care Networks under New York State's 1115 Medicaid Demonstration Waiver. This position involves direct client contact, data management, administrative tasks, and ensuring departmental efficiency by managing new tenant referrals and facilitating team operations.

    Reporting to the Social Care Networks Supervisor, the Intake Coordination Specialist will serve as the primary point of contact for new tenant referrals to Anthos|Home's Medicaid-reimbursable services, the Intake Coordination Specialist will develop relationships with referral sources, screen clients for Medicaid eligibility, and assess unmet health-related social needs (HRSN) using the Accountable Health Communities HRSN Screening Tool. They will also coordinate referrals for Enhanced HRSN Services, including Anthos|Home's housing services.

    This role requires close collaboration with the Anthos Management Team, Managed Care Organizations, Health Homes, health care providers, and community-based organizations to address social determinants of health in alignment with Social Care Network (SCN) guidelines. Additionally, the Intake Coordination Specialist will manage complex documentation, analyze data, and provide administrative support to ensure smooth departmental operations.

    KEY RESPONSIBILITIES

    • Serve as the primary liaison with agencies making tenant referrals and handle escalated inquiries.
    • Develop relationships with referral sources and receive referrals of potentially eligible clients, using the SCNs' IT Platforms.
    • Oversee documentation and data entry into the SCN's IT platforms as well as the Anthos|Home Salesforce case management system, ensuring accuracy, compliance, and confidentiality.
    • Conduct outreach to client by telephone
    • Collect client consent and attestation and review information
    • Confirm client identity and Medicaid enrollment using MEVS or EPACES systems
    • Confirm client desire for and eligibility for enhanced HRSN services
    • Create care plan for client and navigate to services as appropriate
    • Serve as a liaison between clients and HRSN service providers.
    • Submit billing information using the SCNs' IT platforms.

    QUALIFICATIONS & SKILLS

    • 3+ years of relevant experience in housing, public health, human services, or administrative roles.
    • Exceptional interpersonal, organizational, and analytical skills.
    • Strong verbal and written communication skills.
    • Familiarity with the NYC housing market and voucher system is highly desirable.
    • Proficiency in MS Word, Excel, PowerPoint, and Salesforce.
    • Ability to analyze and present data effectively to support decision-making.

    COMPENSATION & BENEFITS

    • Competitive starting salary: $65,000 - $70,000 (dependent on experience).
