    Key Responsibilities

    Referral Management:

    Serve as the first point of contact for hospitals and other referral sources.

    Coordinate and manage the receipt of patient referrals, ensuring all necessary information is obtained.

    Patient Intake and Assessment:

    Conduct comprehensive intake assessments to gather patient demographics, medical history, and required documentation.

    Verify patient eligibility and service needs in accordance with regulatory guidelines and internal protocols.

    Communication & Coordination:

    Liaise with hospital staff, physicians, and referral coordinators to clarify referral details and address inquiries.

    Collaborate with internal teams to schedule assessments, arrange care transitions, and coordinate follow-up care.

    Documentation & Compliance:

    Accurately enter and update patient records in electronic medical records (EMR) systems.

    Ensure compliance with HIPAA and other applicable healthcare regulations.

    Maintain organized documentation and reporting systems to track intake progress and outcomes.

    Insurance & Financial Coordination:

    Assist in insurance verifications, pre-authorizations, and the coordination of patient financial responsibilities.

    Quality Improvement:

    Monitor referral trends and intake processes, suggesting improvements to enhance service efficiency.

    Provide regular feedback and reports to management on intake performance and referral outcomes.

    Relationship Building:

    Develop and nurture strong working relationships with hospital staff and other referral sources.

    Represent Virginia Home Care Services at external meetings, conferences, and networking events as needed.

    Technical Skills:

    Proficiency with electronic medical records (EMR) systems and intake management software.

    Familiarity with healthcare regulations, HIPAA compliance, and insurance verification processes.

    Core Competencies:

    Excellent verbal and written communication skills.

    Strong organizational skills and meticulous attention to detail.

    Ability to multitask, prioritize, and work effectively under pressure.

    Demonstrated problem-solving abilities and a proactive approach to overcoming challenges.

    Working Conditions

    Environment:

    This role is primarily office-based with occasional travel to hospitals or referral source locations.

    Schedule:

    Standard operating hours are Monday through Friday, with flexibility required based on referral needs and hospital schedules.
