    Job Description

    The Integrated Health and Outreach Program Coordinator manages an integrated clinical and community-based program, including screening, testing, treatment, and referral for HIV, hepatitis C, sexually transmitted infections, other transmittable diseases, and substance use disorders. The coordinator provides overall administrative management, direction of program activities, and ongoing support for clinical software applications in electronic medical record systems.

    KEY ACCOUNTABILITIES:

    1. Provides direction of program deliverables to meet goals and objectives in alignment with NATIVE HEALTH’s mission.
    2. Provide administrative oversight of the program.
    3. Coordinate integrated services with clinical staff, behavioral health department, other departments, and partners.
    4. Support the capacity of clinical software applications to improve service delivery of health and program services.
    5. Supports the organization as needed.

    Qualifications

    QUALIFICATION AND EXPERIENCE:

    1. A bachelor’s degree in public health, social work, or other related fields.
    2. Minimum of two years of experience in HIV and STI prevention or other community health programs, preferably within a community health or public health setting.
    3. Bilingual (Spanish/English) preferred, not required.
    4. Master’s degree in public health or similar field preferred.
    5. Experience in HIV and STI programs preferred.
    6. Experience in NextGen preferred.
    7. Willing to travel and work flexible hours, including evenings and weekends.

    Standard Employment Requirements:

    1. Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
    2. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
    3. Must pass substance abuse testing upon employment and submit to random testing during employment.
    4. Must provide an updated immunization (IZ) record at the time of employment.
    5. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
    6. Must obtain BLS certified within (90) calendar days of employment and maintain certification during employment.

    1. Knowledge of HIV, sexually transmitted diseases, viral hepatitis, health promotion, and behavioral health, including prevention, case management, treatment, and referral processes.
    2. Excellent organizational and project management skills.
    3. Strong interpersonal and communication skills, with the ability to collaborate effectively with diverse stakeholders and community members.
    4. Proficiency in data collection, management, and analysis.
    5. Ability to develop and implement clinic and outreach strategies.
    6. Knowledge of grant management and reporting requirements.
    7. Ability to work independently and make informed decisions.
    8. Ability to provide a positive team-oriented environment.
    9. Customer service oriented. Ability to handle difficult situations.
    10. Ability to establish and maintain cooperative relationships with medical staff, representatives of other agencies, clients, and the general public.
    11. Effective verbal and written communication skills.
