Job Summary: We are seeking a compassionate and dedicated Case Manager to join our team and make a meaningful difference in the lives of those we serve.
As a Case Manager at Infinity Care Plus, you will be responsible for coordinating and managing client care, ensuring that individuals receive the appropriate services and support they need. You will work closely with clients to develop personalized care plans, connect them with community resources, and monitor their progress. The ideal candidate will have strong interpersonal skills, a deep understanding of community resources, and a commitment to advocacy and client empowerment.
Key Responsibilities:
Conduct comprehensive assessments of clients' needs, strengths, and goals.
Develop and implement individualized care plans in collaboration with clients.
Provide ongoing support and case management services to clients, including regular check-ins and progress monitoring.
Advocate for clients and connect them with appropriate community resources, services, and support networks.
Maintain accurate and up-to-date case files, documentation, and reports.
Collaborate with other professionals, agencies, and service providers to ensure coordinated and effective care.
Participate in team meetings, training sessions, and professional development opportunities.
Stay informed about changes in community resources, services, and best practices in case management.
Qualifications:
Bachelor's degree in Social Work, Psychology, Human Services, or a related field (Master's degree preferred).
Minimum of 2 years of experience in case management or a related field.
Strong understanding of community resources, social services, and support networks, with experience supporting individuals affected by homelessness, mental health challenges, or substance abuse.
Knowledge of trauma-informed care or motivational interviewing techniques (preferred).
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and collaboratively within a team.
Proficiency in Microsoft Office and case management software.
Valid driver’s license and reliable transportation.
