    Job Summary

    Our cae management group seeking multiple compassionate and professional Patient Engagement Specialist (ER Liaison) to work within a local hospital’s emergency room (ER). The ER Liaison will be responsible for identifying and engaging eligible accident patients as they enter the ER, providing immediate guidance on available resources, and facilitating the initial steps of their care journey. While this role is focused solely on the ER, the liaison will work closely with our external case management team, who will coordinate patient care and legal referrals as needed.

    Please note: This is not a hospital employee role but a third-party position with a flexible schedule, including nights, overnights, holidays and weekends. Employees looking to work all shifts are encouraged to apply.


    Key Responsibilities:

    Patient Identification and Engagement: Identify eligible accident patients in the ER, initiate contact post-stabilization, and provide an overview of the follow-up care services available within the hospital network.

    Cross-Department Collaboration: Collaborate with the hospital’s care team to ensure a smooth handover of patient information for ongoing external care coordination, including medical and legal support.

    Educational Support: Provide patients with resources and information on the next steps in their care journey, including follow-up appointments, available specialists, and legal assistance if requested.

    Documentation and Compliance: Accurately document all interactions within our system, ensuring data compliance with hospital policies, privacy regulations, and patient confidentiality standards.

    • Seamless Workflow Integration: Work closely with ER staff and the case management team to integrate patient capture workflows effectively, providing a clear handoff for external follow-up care.

    Required Skills and Qualifications

    Hard Skills

    Healthcare Knowledge: A solid understanding of healthcare practices in an emergency room setting and the importance of follow-up care coordination.

    Data Documentation: Experience in using electronic medical records (EMR) or other patient documentation systems within a healthcare setting.

    Interdepartmental Coordination: Ability to coordinate effectively with both ER staff and external case management professionals to ensure comprehensive patient follow-up.

    • Legal and Insurance Awareness: Familiarity with insurance processes and legal services associated with accident-related care, with an understanding of how to direct patients to resources effectively.

    Qualifications

    Previous experience as a Care Coordinator or in a similar position is preferred

    Certification as a medical assistant is preferred
