Job Description
ASSIGNED SHIFT: 7:00 a.m. - 3:00 p.m. or 3:00 p.m. - 11:00 p.m. (4 days on, 2 days off rotation)
ESSENTIAL FUNCTIONS: In charge of the designated nursing functions during his/her shift. Provides a wide variety of resident care activities and services including medication administration, direct care, individual and group socialization, (housekeeping, laundry, dietary services) as needed essential for health and comfort of the resident in accordance with their assessment, choices and person-centered service plan. Supervises the day-to-day activities provided by AMIs/CNA’s.
ADDITIONAL FUNCTIONS:
Participates in developing, implementing, and revising of the resident’s person-centered service plan.
Prepares and administers medications and treatments as ordered by physician in accordance with person centered service plan and evaluates resident’s responses in accordance with ARCP Level 4
Coordinates monthly pharmacist medication reviews with Nursing Coordinator and/or Nursing Director
Assist with monthly physician orders, pharmacy and medication review and documentation. Enters orders into EMAR in accordance with physician’s orders
Assists residents/responsible party in ordering or reordering their medications
Documents nursing activities accurately and timely & keeps medical records current
Identifies and reports changes in residents’ status to physicians, responsible parties
Coordinates and participates in admission, transfer, and discharge of residents
Communicates resident problems, needs, complaints, or other concerns with Nursing Director in a timely manner
Responsible for providing direct care as needed
Ensures and assist when needed with other roles performing housekeeping, laundry and dietary task
Responds to emergency situations and obtains assistance as necessary. Performs cardiopulmonary resuscitation
Identifies, reports, and documents incidents/accidents & participates in investigation of incidents
Assigns specific duties to the AMIs on his/her shift and provides adequate supervision of those AMIs to ensure completion of these assignments
Accepts and gives shift report to/from nurses and AMIs
Participates in AMI evaluations and disciplinary action as necessary
Participates in fire drills and in-service
Orients new employees as assigned
Assures resident’s rights are protected; protects residents from abuse or neglect
Practices and assures AMIs adhere to infection control policies and procedures
Adheres to professional code of ethics, facility policies and procedures, and regulatory guidelines
May be assigned to arrange transport of residents to medical appointments
Other related projects, duties and responsibilities may be assigned as deemed necessary.
SUPERVISORY RESPONSIBILITIES
Supervises AMIs
Functions as charge nurse during shift
Documents and reports any violation of The Glen standards to supervisor.
QUALIFICATIONS:
Licensed to practice in Louisiana
LTC experience preferred
First Aid and CPR Certification
Good communication & leadership skills
Ability to handle multiple priorities and manage stress
Computer skills appropriate to job requirements and EHR.
Valid driver’s license and ability to operate motor vehicle
PHYSICAL DEMANDS:
Appropriate physical and mental health to adequately perform assigned duties
Ability to pull, push up to 90 pounds, lift up to 50 pounds
Emotional stability and maturity
Visual and hearing acuity to adequately perform assigned duties
Turning, stooping, bending, and lifting of residents; movement of small equipment and appliances. Significant sitting, standing & walking for extended periods
Finger and hand dexterity to operate small equipment and perform skilled tasks
