Job Summary:
Eastbrook Healthcare Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team on the day shift. In this role, you will provide essential nursing care to our patients, supporting their health and well-being in a warm and welcoming environment. Key Responsibilities:

- Deliver high-quality nursing care to patients, including administering medications, monitoring vital signs, and assisting with daily living activities.
- Collaborate with registered nurses and healthcare providers to develop and implement individualized care plans.
- Document patient progress and maintain accurate medical records in accordance with facility policies and procedures.
- Assist with patient admissions, transfers, and discharges, ensuring a smooth transition for patients and their families.
- Educate patients and their families on health management, medication administration, and follow-up care.
- Participate in team meetings, contributing to discussions on patient care strategies and improvements.
- Ensure compliance with all health and safety regulations, maintaining a clean and organized work environment.

Qualifications:

- Current and valid LPN license in Ohio
- Completion of an accredited Practical Nursing program.

- Ability to work effectively in a fast-paced environment and manage multiple priorities.
- Basic proficiency with electronic health records (EHR) and other healthcare technology.
