    Job Description: As a Home Health LPN, you will play a pivotal role in delivering high quality skilled nursing care to our patients in their homes. You will work closely with the RN Case Manager to ensure the highest level of patient care and achieve optimal outcomes. Your skilled nursing home visit responsibilities will include:

    Administering prescribed medications and treatments as per physician's orders

    Assisting with wound care, catheter care, and other specialized procedures

    Monitoring and documenting patient progress, maintaining accurate and up-to-date records

    Educating patients and their families on health management, medications, and treatment plans

    Collaborating with registered nurses, therapists, and other healthcare professionals to ensure seamless care coordination.

    Communicating effectively with physicians and other members of the healthcare team

    Adhering to all regulatory guidelines and maintaining patient confidentiality

    Monitoring and documenting patient progress

    Compliant and timely charting/ documentation of skilled nursing visits

    Qualifications:

    Active Licensed Practical Nurse (LPN) license in the state of FL

    Minimum of 1-2 years of LPN experience; home health experience is preferred

    Strong clinical skills and the ability to provide compassionate patient-centered care

    Proficiency in administering medications and performing nursing procedures

    Excellent communication and interpersonal skills

    Ability to work independently and as part of a team

    Comfortable using electronic health records and other technology tools

    Reliable transportation and a valid driver's license
