Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to individuals receiving private home health services under DODD guidelines. The ideal candidate will have a passion for working with individuals with developmental disabilities, strong clinical skills, and the ability to work independently while adhering to care plans and regulatory requirements.
Key Responsibilities
Provide direct nursing care based on the individual's Plan of Care (POC) and physician orders.
Administer medications, treatments, and therapies as prescribed.
Monitor and document the individual's health status, including vital signs and any changes in condition.
Assist with activities of daily living (ADLs) as needed.
Educate individuals, families, and caregivers on health conditions, medication administration, and treatment plans.
Maintain accurate and timely documentation in compliance with DODD regulations and agency policies.
Collaborate with RNs, physicians, caregivers, and interdisciplinary teams to ensure coordinated care.
Follow infection control and safety protocols to provide a safe environment for the individual.
Respond to emergency situations appropriately and follow agency protocols.
Qualifications
Current and active Licensed Practical Nurse (LPN) license in the state of Ohio.
Experience working with individuals with developmental disabilities (preferred but not required).
Knowledge of DODD regulations and Medicaid waiver services (preferred).
Valid CPR/BLS certification.
Ability to pass a background check and drug screening per state and agency requirements.
Strong clinical assessment, critical thinking, and communication skills.
