Fortcom is currently looking to hire Part-Time LPN for Boston, Dorchester, Roxbury, Brockton, Lynn, Lowell, New Bedford, Worcester, Brockton areas of Massachusetts to perform home visits to clients of Critical Skilled Nursing program.

Job Responsibilities:

Complete and coordinate all applicable clinical assessments and clinical evaluations

Develop and review on an ongoing basis each member’s AFC plan of care

Training, evaluating, and supervising AFC caregivers in conjunction with Program Director

Reviewing the physician summary

Monitoring the health status of all clients to ensure that all needed care is properly delivered

Reporting changes in the health status of any member to the client’s physician

Provide individualized on-site nursing care to clients in the comfort of their home

Use a comprehensive and client individualized Plan of Care with innovative approach.

Education/Experience:

LPN license (pass NCLEX-PN exam)

Must be fully licensed RN by Mass Board of Registration

At least two years of previous nursing experience

Bilingual (Spanish or Haitian Creole) is a plus
