    Licensed Practical Nurse - LPN Essential Functions:

    • Conduct Home Health Aide skills trainings, competency evaluations, HHA introduction and 90 day evaluations
    • Complete initial client assessment, plan of care, 90 day reassessments, and ongoing evaluations of the GAFC setting
    • Partake in both care plan creations and reviews, along with discharge plans
    • Assess and address motivational, psychosocial, and environmental concerns
    • Maintain and update client charts, medical records and submit all relevant information to authorized organizations
    • Completing notes corresponding with each on-site visit or encounter, or more often as the member's condition warrants
    • Document health changes and communicate to the client's Case Manager and Program Manager in a timely manner
    • Review all paperwork and plans of care to ensure program compliance, updating information as needed
    • Performs any other additional duties that may be required by management at any time

    Licensed Practical Nurse - LPN Requirements:

    • Travel is required
    • Valid drivers license and reliable transportation required
    • Licensed Practical Nurse - LPN Massachusetts license required
    • Minimum of 1 year experience in an acute or home health care setting or comparable work experience preferred
    • Fluency in both English & second language preferred
    • Case management, social services, and/or community health worker experience preferred
    • Past experience working with behavioral health population is a plus
    • Strong communication, writing, organizational, and time management skills
    • Solution oriented with the ability to multitask
