    Pay rate: $31.00/hour

    In this role you will:

    • Manage the care of clients using established protocols, ensuring compliance with all applicable policies and procedures

    • Provide health care and education including but not limited to administering immunizations to infants, children, and adults; lead testing and case management; tuberculosis testing and case management.

    • Investigate reportable communicable diseases; provide education to infected clients and their close contacts, submit appropriate specimens to lab; complete and file epidemiological reports; and submit reports to IDOH. Provide case follow-up as needed.

    • Investigate, document, and provide follow-up for complaints regarding health care delivery.

    • Serve as case manager for pediatric clients with elevated lead levels, providing clients/families with appropriate education on lead poisoning and proper nutrition, repeating lead level tests, conducting home visits/environmental assessments to investigate the source of lead poisoning, and making referrals to appropriate agencies and health care providers to ensure necessary steps are taken to reduce exposure to lead. Complete case documentation in compliance with Department policy.

    • Educate patients regarding tuberculosis (TB), administers TB tests and reads results according to American Lung Association guidelines. Identify and treat people who have active, latent, or suspected tuberculosis (TB), assuring persons with active or latent TB complete appropriate therapy.

    • Conduct TB contact investigations, including locating, identifying, and screening people who have been in contact with TB and providing education/information as appropriate.

    • Collect sputa, analyze results of labs and x-rays, and report results to Indiana State Department of Health (IDOH) TB Control and primary physicians.

    • Serve as TB educator/consultant, providing education, guidance, and basic training to health care providers.

    • Maintain current records and documentation on cases and services provided, enters TB clinic data in computer, and completes and submits required reports to IDOH TB Control and supervisor as appropriate.

    • Administer TB skin tests or IGRAs.

    • Identify and provide latent TB infection case management, including creating charts, forwarding reports to IDOH, and providing medications throughout therapy.

    • Offer support to TB clients, providing social, emotional and/or economical information and/or resources as necessary. Provide ongoing reassessment, analysis, and evaluation for termination of services.

    • Advise patients on needed routine vaccinations for various age groups based on Centers for Disease Control and Prevention (CDC) guidelines.

    • Conduct and/or assist with infectious disease testing, including but not limited to sexually transmitted infections, hepatitis, HIV, and AIDS.

    • Review patient records to ensure accurate immunizations are being administered; prepare and administer immunizations for injection using sterile techniques.

    • Review with patients what immunizations are being administered and provide information regarding immunizations, possible side effects, and remedies for side effects. Monitor patients for signs of anaphylactic reaction. Review patient assessments for contraindications.

    • Educate the public on vaccine preventable diseases and periodically conduct off-site vaccination clinics.

    • Maintain communication with physicians’ offices, school nurses, and other health professionals, providing information and assistance and/or recommending resources as appropriate.

    • Provide health care services to the public and clinic employees, responding to questions, taking blood pressure readings, and presenting general health management and smoking cessation education.

    • Coordinate and conduct public health fairs and immunization clinics, providing blood pressure readings, diabetes screening, immunizations, and information/instruction as needed.

    • Assist with the development of policies, procedures and new programs related to the Nursing Division.

    • Respond to telephone calls and questions from members of the public, including scheduling appointments, making referrals, counseling, and providing health-related information as appropriate.

    • Maintain current records and documentation on services provided, completing a variety of weekly, monthly, and annual reports as required by supervisor.

    • Serve on local boards, committees, and task forces, attending health-related meetings as necessary. Prepare and conduct educational presentations for local schools and businesses as requested.

    • Assist police, fire departments, local government agencies and health clinics on bioterrorism preparedness plans and procedures.

    • Attend in-service programs and professional education workshops/training seminars as required.
