    Job Description

    We are seeking an experienced and dedicated LPN to join our home health and private duty division. The ideal candidate will see and manage the clinical patients for both our skilled and nonskilled agencies, ensuring exceptional patient care and compliance with healthcare regulations.

    Key Responsibilities

    Staff Management:

    Hire, train, and evaluate private duty nurses and caregivers.

    Schedule and coordinate staff to meet patient needs efficiently.

    Patient Care Oversight:

    Develop and implement individualized care plans.

    Conduct regular assessments to monitor patient progress and adjust care plans as needed.

    Ensure all clinical practices adhere to established medical guidelines and standards.

    Regulatory Compliance:

    Ensure all operations comply with healthcare regulations and standards.

    Maintain accurate and up-to-date patient records and documentation.

    Coordination and Communication:

    Act as a liaison between patients, families, and healthcare providers.

    Coordinate with other healthcare professionals to provide comprehensive patient care.

    Quality Improvement:

    Identify areas for improvement in patient care and operational efficiency.

    Implement quality improvement initiatives to enhance patient outcomes and satisfaction.

    Crisis Management:

    Address and resolve issues or emergencies in patient care.

    Provide guidance and support to staff during challenging situations.

    Education and Training:

    Offer ongoing education and training opportunities to staff to keep them updated on best practices and new healthcare developments.

    Qualifications

    LPN with an active state license

    Minimum of 1-3 years of clinical experience in a home healthcare or private duty setting.

    Strong leadership, organizational, and communication skills.

    Ability to work collaboratively with a diverse team.

    Knowledge of healthcare regulations and standards.
