Job Description:
We are looking for a reliable LVN/LPN (Licensed Practical Nurse/Licensed Vocational) to administer direct patient care, administer approved medications, perform treatments, make accurate nursing observation, and chart on the patients' medical record in a professional and considerate manner. Duties are performed under the direction of the Case Manager and/or Patient Care Manager. LVN/LPN professionals must be first and foremost qualified and compassionate. The ideal candidate preference will have at least one year experience as a nurse in a health care setting. Home health experience preferred. We offer competitive pay and flexible schedule.
Educational Requirements:
graduate of an approved program of vocational/practical nursing
Licensing/Certification/Registration Requirements:
current Licensed Vocational/Practical Nurse (LVN/LPN) from the Texas Vocational Nurse Examiners
current CPR
BLS certification preferred.

Physical Requirements:

able to operate a motor vehicle and travel to prospective patient’s residences.
Responsibilities:
assists in assessing patient and family deeds through the recognition and reporting of signs and symptoms and nursing observation to the assigned Case Manager
assists in assessing the health status of assigned patients.
complies with the established nursing assessment guidelines/policies.
utilizes the written plan of care for implementing nursing interventions necessary to accomplish the established goals.
complies with general performance standards of licensed vocational nurse. Demonstrates flexibility in response to unexpected changes in work volume, emergencies, staffing, or scheduling changes.
must have a reliable means of transportation, proof of financial responsibility (car insurance and a valid state driver’s license)
