Job Title: Lead Care Manager Organization: United and Guided Location: Sacramento, CA Job Type: Full-Time
About United and Guided
United and Guided (UAG) is a community-based healthcare organization dedicated to providing trauma-informed, whole-person care to underserved communities in Sacramento. Through Enhanced Care Management (ECM), Community Supports, Mental Health Peer Support, Doula Care, and Day Habilitation, UAG ensures that Medi-Cal recipients receive coordinated, person-centered services that improve long-term health and stability.
We are seeking a Lead Care Manager to oversee care coordination efforts, provide direct support to clients, and supervise a team of Peer Support Specialists and Community Health Leaders. The ideal candidate is experienced in case management, behavioral health, or community health services with a strong understanding of CalAIM, Medi-Cal benefits, and social determinants of health (SDoH).
Key Responsibilities
Care Coordination & Case Management
Develop individualized care plans addressing medical, behavioral, and social needs.
Coordinate ECM and Community Supports services with Medi-Cal Managed Care Plans (MCPs) such as Anthem, Kaiser, and Health Net.
Serve as a liaison between clients, providers, social services, and community resources.
Provide crisis intervention and advocacy for individuals experiencing homelessness, mental illness, or justice involvement.
Team Leadership & Supervision
Supervise Peer Support Specialists, Community Health Workers (CHWs), and Care Coordinators.
Ensure adherence to trauma-informed, culturally competent, and evidence-based care practices.
Maintain compliance with DHCS, CalAIM, HIPAA, and Medi-Cal regulations.
Client Advocacy & Social Determinants of Health (SDoH)
Identify barriers to care such as housing instability, food insecurity, and unemployment.
Assist with public benefits navigation (Medi-Cal enrollment, disability applications, housing assistance).
Support clients in self-advocacy and independent living skills.
Preferred Qualifications & Experience
Education & Experience
Bachelor’s degree or higher in Social Work, Public Health, Psychology, Nursing, or a related field.
OR Associate’s degree and 10+ years lived experience recovering from mental health institutionalization, homelessness, or incarceration.
Professional Certifications & Licensure (Preferred but not required)
California Certified Peer Support Specialist (CPSS)
Licensed Clinical Social Worker (LCSW)
Marriage and Family Therapist (MFT)
Registered Nurse (RN) or Licensed Midwife (LM)
Medi-Cal Rendering Provider Certification
Key Skills & Competencies
Strong knowledge of CalAIM, ECM, and Community Supports services.
Experience working with justice-involved individuals, unhoused populations, and those with serious mental illness (SMI) or substance use disorders (SUDs).
Ability to lead a multidisciplinary team and navigate complex care systems.
Bilingual candidates (especially Spanish-speaking) are highly encouraged to apply.
