Location: Destiny's Caring Hands LLC – 220 W. Edison Rd, Mishawaka, IN 46545 Schedule: Full-Time and Part-Time Available Pay: $20–$30 per hour (based on experience)

Key Responsibilities
Integrated Health Care Coordination (IHCC) – Hybrid Services
Provide professional social services and healthcare coordination based on physician orders
Support patients and families in navigating the challenges of illness and treatment
Conduct care coordination through phone calls, virtual check-ins, and two required in-person visits per client per month (1 hour minimum)
Assist with medication reconciliation, chronic condition education, and goal-setting
Collaborate with physicians, families, and care teams to ensure continuity of care
Maintain compliance with all state, federal, and agency policies through accurate documentation
Adult Day Center – On-Site Responsibilities
Conduct health assessments, administer medications, and monitor vital signs
Support the development and execution of individualized care plans
Work alongside a multidisciplinary team to provide social, emotional, and clinical support
Ensure a safe and engaging environment for adult day program participants
Observe and document changes in client health status and report to the supervising RN
Qualifications
Active and unencumbered LPN license in the State of Indiana
CPR and First Aid certification (or willingness to obtain)
Minimum of one year of experience working with elderly, medically complex individuals, or persons with disabilities preferred
Passion for patient advocacy and a client-centered approach to care
Ability to work independently in a hybrid role and maintain compliance with all regulatory requirements
Familiarity with Indiana Medicaid Waiver programs (A&D, TBI, IHCC, SFC) is a plus
Proficiency in electronic documentation, HIPAA compliance, and care coordination platforms
