Job Summary:
Santa Fe Heights HCC is seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team for the night shift (11 PM to 7 AM). The successful candidate will play a vital role in delivering high-quality patient care, ensuring a safe and therapeutic environment, and working collaboratively with a multidisciplinary team. This position requires strong clinical skills, attention to detail, and a commitment to providing exceptional nursing care.
Key Responsibilities:
- Provide direct patient care, including administering medications, treatments, and therapies as prescribed by a physician.
- Monitor and record vital signs, changes in patient condition, and response to treatment.
- Assist in personal care activities, including bathing, grooming, and feeding, while promoting patient dignity and independence.
- Collaborate with registered nurses and other healthcare professionals to develop and implement individualized care plans.
- Educate patients and their families on health-related topics, procedures, and self-care techniques.
- Ensure compliance with facility policies, procedures, and regulatory requirements.
- Maintain accurate and up-to-date patient records and documentation in accordance with HIPAA regulations.
- Participate in shift handovers and communicate effectively with team members regarding patient care.
- Assist in maintaining a clean, safe, and orderly environment for patients and staff.
Qualifications:
- Current Licensed Vocational Nurse (LVN) license in the state of California.
- Graduate of an accredited vocational nursing program.
- Previous nursing experience in a healthcare environment preferred.
- Strong clinical assessment skills and the ability to prioritize tasks effectively.
- Excellent communication and interpersonal skills, with a compassionate and patient-centered approach.
- Ability to work collaboratively in a team-oriented environment and adapt to changing situations.
- Basic life support (BLS) certification is required.
Physical Requirements:
- Must be able to lift, transfer, and position patients as necessary.
- Ability to stand, walk, and bend for extended periods.
- Must be able to perform tasks that require fine motor skills.
