    Job Description:

    The Licensed Behavioral Health Therapist is responsible for meeting the mental health needs of the organization’s patients by providing 1:1 and/or group therapy in the medical clinic, emergency shelters, MMU and/or directly in the community as part of the community-based street medicine program. The Behavioral Health Therapist will work as part of an interdisciplinary team that serves clients experiencing homelessness and work as a bridge to transition clients to long-term mental health care.

    Responsibilities for the position include but are not limited to:

    Initial assessment, treatment plans, treatment plan reviews and updates, progress notes and all admission and referral paperwork.

    Engage clients in person-centered needs assessments to support linkages and referrals to other ongoing mental health services at appropriate levels of care.

    Develop an individualized treatment plan in collaboration with the client based upon the needs identified in the assessment interview.

    Help restore social conditions to enhance the patients’ capacity to meet personal and social needs.

    Provides evidence-based mental health therapy interventions when providing individual and/or group therapies.

    Shares information about community resources to meet client needs and assists clients in obtaining them.

    Connects clients to aftercare and community-based, long-term mental health treatment through detox, inpatient or outpatient services as indicated.

    Acts as advocate and liaison of client in securing other resources when appropriate.

    Coordinates case planning and progress internally through team meetings and individual contacts with other disciplines.

    Engage in case consultation with community providers as needed. Assess, evaluate and discuss progress, updates and interventions in the treatment plan as needed.

    Engage in crisis intervention services for individuals experiencing acute mental health distress.

    Experience:

    1-3 years experience working with an underserved population preferably persons experiencing homelessness.

    Experience conducting individual and group therapy.

    Experience working with trauma, domestic violence, alcohol and substance abuse, and co-occurring disorders a plus.

    Experience in ICD-10 coding, Epic & Care Connect.

    Knowledge of various counseling modalities including cognitive behavioral therapy

    Education

    Master’s degree in counseling, Psychology, or related field, preferred.

    Bachelor’s degree required.

    Required Licensure/Certification Skills:

    Current licensure to practice as a Mental Health Counselor/ Behavioral Health Therapist in NYS, required.

    Credentialed Alcohol and Substance Abuse Counselor (CASAC), preferred.

    Job Type: Full-time Working Conditions:

    In-person (Clinic, Shelter, Steet, and Mobile Medical Units)

    Monday through Friday

    8:00 AM - 4:30 PM, with some flexibility
