    Harmonia Collaborative Care is seeking a Licensed Clinical Social Worker (LCSW) to join our dedicated team of mental health professionals in Hamburg, NY. We are committed to providing compassionate, patient-oriented care to improve the quality of life for individuals in our community.

    Job Description:

    As a Licensed Clinical Social Worker at Harmonia Collaborative Care, you will have the opportunity to make a meaningful impact on the lives of individuals struggling with mental health challenges. You will work collaboratively with our multidisciplinary team to provide high-quality mental health services in a supportive and balanced work environment.

    Responsibilities:

    - Conduct comprehensive mental health assessments

    - Develop and implement individualized treatment plans

    - Provide evidence-based therapeutic interventions

    - Collaborate with other healthcare professionals to ensure holistic care

    - Maintain accurate and timely clinical documentation

    - Participate in team meetings and case consultations

    - Engage in ongoing professional development

    Qualifications:

    - Master's degree in social work from an accredited institution

    - Current and valid LCSW license in New York State

    - Minimum of 2 years post-licensure clinical experience

    - Proficiency in evidence-based therapeutic modalities (e.g., CBT, DBT, MI)

    - Strong interpersonal and communication skills

    - Ability to work effectively in a team-oriented environment
