    Location: Washington, DC

    Schedule:

    Monday – Friday, 9:00 AM – 5:30 PM (5x8-hour shifts)

    Attire: Business Casual

    Position Overview:

    Seeking a Licensed Independent Clinical Social Worker with 2+ years of experience in pediatric care, particularly for medically complex cases requiring technology-assisted care. This role focuses on providing holistic social work services to pediatric patients and their families, fostering an empowering environment, and supporting patient care coordination.

    Required Certifications/Licenses:

    BLS (Basic Life Support)

    LCSW (Licensed Clinical Social Worker)

    DC License

    Key Responsibilities:

    Care Coordination:

    Document information to facilitate patient care coordination.

    Actively participate in care conferences.

    Refer patients and families to additional resources and specialized disciplines as needed.

    Patient Assessment:

    Conduct initial evaluations and comprehensive biopsychosocial assessments.

    Develop individualized care plans tailored to each child’s unique needs.

    General Skills & Expertise:

    Deep understanding of ADA regulations, CPS protocols, and adoption/foster care systems.

    Proven experience working with pediatric patients with medical complexities.

    Strong skills in both written and verbal communication, particularly in a multidisciplinary team setting.

    Ability to navigate family dynamics, understanding social determinants of health impacting patient care.

    Preferred Skills:

    Case Management Certification (strong positive).

    Ability to problem-solve effectively and engage families in proactive care approaches.

    Organized and detail-oriented, with strong interpersonal and advocacy skills.
