    Managed care company is looking for a  Licensed Occupational Therapist to conduct in home assessments for a long-term (3 year) contract.

    WORK LOCATION:  Travel throughout the Snohomish County Area

    PAY: $65.00

    CONTRACT DATES: ASAP to 9/30/27 (3 year contract)

    HOURS: Mon-Fri, 8:00 - 5:00

    JOB SUMMARY

    Visit approximately 110 clients within Snohomish County three times in a 6-month period. Visit and evaluate qualified clients (age 62+) in their primary residence for home modifications that address the client’s specific goals and needs and promote their full participation in daily life activities. The first visit is the assessment and report used to create a work order. The second visit is to ensure that work that was complete is satisfactory to the client's needs. The third visit is just an assessment follow up. The following responsibilities include, but are not limited to:

    RESPONSIBILITIES

    • During in-home client visits and using an aging-in-place assessment, evaluate clients’ functional abilities and the home environment to with the knowledge to recommend a range of low-cost, high-impact environmental modifications and adaptive equipment used to optimize the home environment and increase independence.
    • Encourage a person-centered approach that motivates and supports older adults as they identify their goals and learn to function safely in their home.
    • Perform client education regarding the connection between accessibility modifications and health and safety.
    • Assess and report any clients who may be experiencing more significant memory loss, that affects their safety of living alone – referring these clients to helpful resources at Homage and beyond.

    QUALIFICATIONS

    • Occupational Therapist License, with aging in place focus.
    • Two years of experience in aging in place accessibility modifications.
    • Demonstrated ability to work well with a diverse population. Must be sensitive to the needs of low-income elderly and disabled persons.
    • Candidate must be able to work with clients in hoarding situations, poor living conditions and resistance to accessibility modifications.
    • Experience with and ability to learn and navigate different program databases; effective working knowledge of Microsoft Office suite (Outlook, Teams, Excel, Word, PowerPoint); comfortable navigating in Microsoft Windows environment and using internet browsers
    • Clean driving record.

    PREFERRED QUALIFICATIONS

    • Experience working with older adults and/or adults with disabilities
    • Experience with Salesforce or similar database
    • Experience working with diverse communities
    • Fluency in other languages in addition to English
