    Under general supervision, evaluates and assesses infants and toddlers in the Babies Can't Wait

    program for multiple counties within the North Central Health District. Implements physical therapy

    services to maximize the child's independent physical functioning. Works as a member of a

    multidisciplinary team and coaches staff and family members in motor development of young children, providing supportive strategies within home and community settings. Supports families based on a

    Primary Service Provider/Coaching Model. Works to adapt materials and arrange environment to

    improve child's participation in daily activities. Maintains documentation of progress in the

    Babies Information and Billing System (BIBS) in accordance with Medicaid guidelines and provides care coordination for families. Provides follow up support and assists with the development of strategies for other team providers; actively provides treatment within the natural environment (home, daycare,

    community settings) for eligible children.

    JOB RESPONSIBILITIES:

    Under broad supervision, assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength and decrease or prevent deformity of patients suffering from

    disease or injury. Helps patients in restoring vocational, homemaking, and daily living skills, as well as general independence, to disabled persons. May serve as a lead worker.

    MINIMUM QUALIFICATIONS:

    Doctorate or professional degree in physical therapy AND current or eligible for Georgia licensure in

    area of assignment AND Three years of clinical experience in area of assignment OR Three years of

    experience at the lower level Phys/Occupational Therapist 2 (HCP041) or position equivalent.

    Preferred QuaHUcatioos

    Knowledge of Part C Program of Individuals with Disabilities Education Act (IDEA). Three years

    experience, extensive knowledge and experience in working with children birth to 3 years. Enrolled a

    an approved provider with Georgia's Medicaid Care Management Organizations (CMOs).
