Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home care team. The ideal candidate will provide essential nursing care to patients in the comfort of their homes, focusing on personalized and high-quality care tailored to each individual's needs. The LPN will collaborate with registered nurses and physicians to ensure the delivery of comprehensive and effective care, supporting patients in achieving optimal health outcomes.

Duties

Administer medications and perform treatments as prescribed by healthcare providers in a home care setting.

Assist with tube feeding and monitor patients’ nutritional intake, ensuring their dietary needs are met.

Conduct thorough patient assessments, documenting vital signs and medical histories with precision.

Collaborate with healthcare teams to develop and implement individualized patient care plans.

Provide prompt and effective care in emergency situations, applying knowledge of home-based emergency protocols.

Maintain sterile techniques to prevent infections during procedures conducted in the home environment.

Utilize medical terminology effectively when communicating with team members and documenting patient information.

Deliver specialized nursing care for pediatric patients, addressing their unique needs in a home care setting.

Apply knowledge of physiology to understand patient conditions and respond appropriately to changes.

Adhere to infectious disease care protocols, ensuring compliance with all home care safety and hygiene standards.

Qualifications

Valid LPN license in the state of New Jersey.

Prior experience in home care is preferred but not required.

Strong understanding of medical terminology and knowledge of physiology.

Experience or training in tube feeding, sterile techniques, and home-based emergency care is highly desirable.

Ability to collaborate effectively as part of a healthcare team while demonstrating excellent communication skills.
