We service (Berks, Bucks, Carbon, Chester, Lehigh, Lancaster, Monroe, Montgomery, Northampton and Schuylkill counties)

We work with your schedule; you get to choose the type of client you want. We have Pediatric and Geriatric clients! Bilingual is a plus!

Responsibilities:

• Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Maintain accurate and up-to-date medical records and documentation.
• Monitor patients for changes in condition and report any concerns to the supervisor.
• Educate patients and their families on promoting healthy living and need to maintain medication regimen

Requirements:

-Valid driver's license and reliable transportation.

• Valid LPN license in the state of employment. -Passion for providing compassionate care -Strong communication and interpersonal skills -Ability to work independently and as part of a team -Previous experience in home care is a plus
