Position Overview:
As a Full-Time Hospice LPN, you will play an essential role in providing hands-on nursing care to our patients while supporting their families throughout their hospice journey. You will work closely with our interdisciplinary team to ensure that every patient receives the comfort, symptom management, and emotional support they deserve.
Position Schedule and Work Hours: Monday to Friday, 0830 to 1730 (8:30AM to 5:30PM) Weekends Off unless the employee personally requests to work weekend hours. (Any Requested Weekend Hours Are Not Guaranteed)
Key Responsibilities:
✔ Provide compassionate, patient-centered care to hospice patients in their homes or care facilities ✔ Administer medications and treatments following hospice protocols ✔ Monitor patient conditions, document observations, and report any changes to the RN Case Manager ✔ Assist with pain and symptom management to ensure patient comfort ✔ Offer emotional support and guidance to patients and their families ✔ Collaborate with the hospice care team, including RNs, social workers, chaplains, and aides ✔ Educate families and caregivers on patient care and end-of-life processes ✔ Maintain accurate and timely documentation in compliance with hospice regulations
Qualifications:
✔ Active LPN license in New Mexico (required) ✔ At least 1 year of experience in hospice, palliative care, senior care, or a related field (preferred) ✔ Strong clinical assessment and critical thinking skills ✔ Excellent communication and interpersonal abilities ✔ A compassionate and empathetic approach to patient care ✔ Ability to work collaboratively within an interdisciplinary team
