Licensed Practical Nurse (LPN)
Job Description
MBI is seeking an experienced LPN (Licensed Practical Nurse) interested in becoming a part of a dynamic team of service-driven health professionals.
The qualified LPN (Licensed Practical Nurse) will care for the sick, injured, convalescent, and disabled under the direction of physicians and registered nurses.
Everyday Responsibilities and Duties:
Gather information about the patient's/clients psychological, physiological, sociological, cultural and nutritional, and spiritual status through interviews with the patient, family, case manager, and/or designated power of attorney.
Collaborates with a team to include patient/client, family, health care provider, ( medical or mental health) case/care manager, non licensed staff to develop a person-centered plan of care of the patient/client
Develops and implements educational plans and/or goals to assist the patient/client in achieving maximum health care benefits.
Promotes and provides patient/client safety through established nursing care processes, the scope of service, and/or regulatory components to include medication storage, medication reconciliations, medication administration, and measuring medication compliance of the patient.
Responds to and reports any changes in the patients/clients condition in a timely manner, documenting any changes in the patient intervention and goals within 24 hours.
Stays informed of state and federal regulations and compliance related to the nursing practice of designated work areas to include but not limited to: behavioral/mental health, developmental disabilities, youth services, home health care services.
Documents nursing assessments and interventions utilizing the established electronic health record within 24 hours of patient/client interaction
Provides follow up and feedback to the patient/client related to their health care concerns and problem
Provides the Physician and/or Nurse Practitioner with medical/nursing information related to patients clients response to medications and/or treatments to include but not limited to side effects of medications
Advocates for the patient/client through the reporting and follow up on patient/client health care issues and concerns
Assist the relevant MBI departments in developing, evaluating, and revising nursing policies and procedures
Maintaining and facilitating HIPAA compliance/Privacy laws
Supports MBIs mission, vision, and value statement

Qualifications
Successfully completed a Practical Nursing Education Program from a recognized institution.
A minimum of 1 years of recent Practical Nursing experience as a licensed nurse in the relevant area to include: Mental/behavioral health, developmental disability, youth/adolescent services, working with the elderly, and/or home care experience
Ability to function in a correctional environment
Valid/Up to date CPR card
Excellent verbal and written communications skills
Electronic Health Records experience ( preferred
