    We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team. The ideal candidate will bring a commitment to resident-centered care and an ability to provide essential clinical support within a collaborative team environment.

    Responsibilities ****

    • Provide daily nursing care and services to residents, including administering medication, monitoring vital signs, and assisting with mobility and personal hygiene needs.
    • Conduct routine assessments and document changes in residents' health status, collaborating with Registered Nurses (RNs) and physicians to develop individualized care plans.
    • Administer prescribed medications, accurately document administration and reactions, and monitor for side effects, ensuring compliance with facility protocols and state regulations.
    • Actively engage with residents, promoting independence while providing assistance with activities of daily living (ADLs) and supporting a positive quality of life.
    • Communicate effectively with residents' families, updating them on residents' progress, changes in health, and responding to questions or concerns.
    • Work closely with interdisciplinary teams, including RNs, therapists, dietitians, and social workers, to ensure holistic, high-quality care.
    • Follow established protocols and policies, including infection control, health and safety guidelines, and regulatory standards.
    • Participate in ongoing training and professional development to stay updated on the latest nursing practices, clinical techniques, and regulatory requirements.

    Requirements

    • Active Licensed Practical Nurse (LPN) license in the state of Texas.
    • Prior experience in a skilled nursing or assisted living facility preferred; new graduates will also be considered.
    • Proficiency in patient care, medication administration, and documentation. Experience with electronic health records (EHR) systems is a plus.
    • Excellent verbal and written communication skills, with the ability to communicate effectively with patients, families, and the healthcare team.
    • Demonstrated compassion, patience, and a resident-focused approach to care.
    • Strong teamwork and collaboration skills, with the ability to contribute to a positive work environment.
    • Strong organizational skills and attention to detail, particularly in following care plans and adhering to safety and documentation protocols.
    • Ability to stand, walk, and assist with resident transfers or other physical activities as necessary.
