We are seeking a PRIVATE DUTY NURSE/ LPN to join our team in the local Edina area! You will be responsible for the assessment, diagnosis, and treatment of assigned patients. We're looking to hire a full time or part time LPN who is competent to perform the following task including but not limited to:
Performs focused nursing assessment of Residents to determine appropriate interventions to maintain or improve the Resident’s health status.
Provides interventions for Residents with episodes of potentially life-threatening instability.
Provides nursing visits and direct care to Residents in the resident's home or assisted living as directed by client Care Plans.
Sets up medications for Residents according to the medication management program, as indicated.
Coordinates medical appointments for Residents, including follow-up with any new orders or changes after hospital visit with direction from Nursing case manager as needed
Accepts and implements prescriber orders with direction as needed
Responds to phone calls from other providers related to Residents with RN direction as needed
Documents, as indicated, in the clinical record
Communicates with families, resident representatives, case managers and other health care providers regarding the Residents as needed
Is aware of and uses good safety practices, including but not limited to:
Body mechanics
Falls prevention
Reporting of defective equipment or hazards
Wiping spills or wet floors immediately
Using Standard Precautions and appropriate infection control measures
Knowledge of emergency procedures
Reporting incidents/accidents to the Clinical Nurse Supervisor or Director on the day of occurrence
Participates in care conferences with other members of the health care team
Maintains absolute confidentiality of all information pertaining to Residents, families, and employees
Promotes a safe and comfortable environment for the Residents
Demonstrates excellent written and verbal communication skills
12 Hour Shifts:
Morning 8 AM - 8 PM
Evening 8 PM - 8 AM
Responsibilities:
Administer nursing care to ill, injured, or disabled patients
Diagnose and establish patient treatment plans
Monitor and report changes in patient symptoms or behavior
Communicate with collaborating physicians or specialists regarding patient care
Educate patients about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records
Provide advice and emotional support to patients and their family members
Qualifications:
Previous experience in nursing or other medical fields
Familiarity with medical software and equipment
Ability to build rapport with patients
Strong problem solving and critical thinking skills
Ability to thrive in a fast-paced environment
