    The qualified Licensed Practical Nurse (PRN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Boost Home Healthcare policy. This is a PRN position.

    What you’ll be doing:

    Implement the nursing care plan for each patient.

    Provide nursing services, treatments and diagnostic and preventative procedures as assigned.

    Initiate preventive and rehabilitative nursing procedures as appropriate.

    Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.

    Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.

    Maintain accurate and complete records of observations, treatments, and care of patients.

    Attend staff meetings as needed.

    Adhere to all HHA, payroll, billing, and documentation policies and procedures.

    Complete assigned training on time.

    Perform other duties as assigned.

    A passion to serve and help others live their best lives possible.

    Graduate of an approved school of practical (vocational) nursing.

    Licensed practical (vocational) nurse in Indiana.

    Minimum of one (1) year as an LPN in the Home Health Care Setting.

    Must have current BLS Certification.

    Valid state driver's license and reliable automobile, current automobile insurance, and willingness to operate personal automobile as needed, based on the nature of the job.

    Successfully pass motor vehicle record screening, drug screening, and background/medical checks.
