We are looking for a full-time Licensed Practical Nurse (LPN) to join our team at Summer Pediatrics, located in North Stamford. Our practice serves a diverse community, and we seek a well-trained, highly motivated nurse who is friendly with children, organized, and eager to help. As a certified Patient-Centered Medical Home, we are committed to providing our patients with the highest standard of care and effectively coordinating their chronic health issues.

Responsibilities:

Nursing care

Phone triage

Coordinate daily huddles when needed

Administer medications and immunizations

Complete Medical Forms

Monitor and report changes in chronic patient

Communicate with collaborating physicians or specialists regarding patient care

Educate patients about health maintenance and disease prevention

Facilitate referrals to other healthcare professionals and medical facilities

Maintain accurate patient medical records

Provide advice and emotional support to patients and their family members

Qualifications:

Previous experience is a plus but not needed

Familiarity with medical software

Ability to build rapport with patients

Strong problem-solving and critical-thinking skills

Ability to thrive in a fast-paced environment
