    Position Overview: We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN at our practice, you will be responsible for providing high-quality nursing care to our patients while maintaining the highest standards of professionalism and patient satisfaction.

    \Skills, Knowledge and Abilities

    Conduct comprehensive patient assessments and develop collaborative treatment plans.

    Promote patient empowerment and self-determination; assist with adherence to treatment plans and self-management goals.

    Monitor, evaluate, and document patient progress.

    Collaborate with outside service providers to coordinate patient treatment and manage referrals.

    Record all interventions in patient charts and produce service delivery and outcome reports.

    Communicate through personal contact, phone, email, and regular mail; return messages promptly.

    Work with Chief Operations Officer and Clinical Director on data collection for outcome measurement reporting.

    Report patient progress in files and to medical staff.

    Collect and prepare laboratory specimens; coordinate with lab companies as needed.

    Perform basic laboratory and point-of-care testing.

    What Expertise You Will Bring

    Oklahoma Licensed Professional Nurse (LPN) License. Must have a current and valid LPN license in the state of Oklahoma.

    Effective Communication. Strong ability to communicate with diverse individuals, including patients, families, and healthcare professionals.

    Problem-Solving and Critical Thinking: Demonstrated skills in problem-solving and critical thinking to provide the best patient care.

    Physical Requirements: While performing the duties of this job, employees are regularly required to sit, walk and stand; talk and hear, both in person and by telephone. Requires sitting, standing and walking for extensive periods of time. Must have strength sufficient to assist with lifting and transferring a patient and perform CPR. Must have sufficient mobility to sit, stand, bend, stoop, and bend down to the floor; ability to move around rapidly; and to move in small, confined areas.
