Tricare Home Health Services Inc. is seeking a Licensed Vocational Nurse (LVN) to join our dynamic office team. This role offers a unique opportunity to contribute to case management, patient coordination, and compliance—ensuring smooth daily operations in a well-established home health agency.

📌 Key Responsibilities:

📝 Case Management & Coordination – Oversee patient care plans, ensure regulatory compliance, and facilitate seamless service transitions. 📂 Daily Operations Support – Assist with chart reviews, documentation, patient follow-ups, and provider communication. 🤝 Patient & Provider Liaison – Act as a bridge between patients, families, and healthcare providers to ensure effective communication and high-quality care. ⚖ Regulatory Compliance & Quality Assurance – Maintain agency standards, ensure accurate paperwork, and support home health initiatives. 📲 Telehealth Support & Virtual Patient Monitoring – Assist with patient outreach, follow-up calls, and remote health monitoring, ensuring patients receive timely care through our new telehealth program.

🔎 What We’re Looking For:

✅ Active LVN License in Texas ✅ Minimum 2 Years of Experience (Home health experience preferred) ✅ Strong Organizational Skills – Attention to detail, ability to multitask, and manage priorities effectively ✅ Excellent Communication & Teamwork – A positive, professional attitude is essential ✅ Tech-Savvy or Willing to Learn – Comfortable using telehealth platforms, EMR systems, and remote monitoring tools ✅ Bilingual (Spanish) Preferred but not required
