Job Summary The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include the following the plan of care providing treatments, and working collaboratively with the members of the members of the team to help meet the patient’s needs.
Responsibilities and Duties
· Develop a Plan of Care and interventions for assigned patients are met while working closely with the Interdisciplinary Group, patient, family and attending physician.
· Provides accurate and timely documentation consistent with the plan of care
· Provide comprehensive nursing evaluations and assessments.
· Responsible for the coordination of care for the patient and family.
· Assist the scheduling team to ensure the patient’s care schedule is accurate
Handle emergency patient situations such as hospitalizations and necessary behavioral interventions.
· Service coordination with hospital social worker and other key community
resources, including CBAS, IHSS, MSSP, community resources, and waiver programs.
· Computer skills to document all services in electronic healthcare system/ EMR
· Work closely with medical professionals to advocate for patient’s health
needs, family or caregivers while building and strengthening relationships
· Engage in effective transition of care from inpatient/SNF setting to home or
different levels of care
· Supervision of the field staff to ensure patient’s needs are met.
· Drive to client homes and or facilities when necessary
· Properly document and track activity to ensure proper billing
· Participate in coordination of home health services
· Uses equipment and supplies effectively and efficiently
· Provide field supervision to LVN’s and Home Health Aides assigned
· Performs other duties as assigned
