    We are seeking experienced Advanced Practice Providers Nurse Practitioners in Pittsburgh to fill a Locum Tenens position focused on providing personalized, compassionate care to patients living with chronic kidney disease (CKD) and end-stage renal disease (ESRD/ESKD). This role involves working as part of an integrated care team to deliver high-quality care in both patient homes and virtual environments.

    Key Responsibilities

    • Conduct comprehensive wellness exams and assessments in patient homes and virtually.
    • Educate patients and families on preventive care and available health programs.
    • Document patient histories, assessments, and care plans accurately.
    • Coordinate care with primary care providers, specialists, and ancillary services.
    • Prescribe medications, order diagnostic tests, and collaborate with physicians.
    • Participate in integrated care team meetings to enhance patient outcomes.
    • Close gaps in care by scheduling necessary diagnostic tests and providing educational resources.

    • Schedule: Flexible locum tenens role, requiring consistent availability.
    • Patient Volume: Manage a defined patient panel with a focus on quality care.
    • Duration: Short- and long-term contracts available.
    • Focus: Ambulatory care—no emergency or hospital coverage required.

    Position Requirements

    • Licensure: Active and unrestricted NP or PA license, board-certified (NP: ANCC/AANP; PA: NCCPA).
    • Experience: 2+ years of direct patient care, ideally in managed care or wellness visits.
    • Current DEA certification and valid driver’s license with reliable transportation.
    • Proficiency with EMR systems and point-of-care technology.
    • Knowledge of federal and state healthcare regulations, including HIPAA.
    • Bilingual (English/Spanish) highly preferred.
